Pathological tooth abrasion is one of the most topical issues for the moment, which is neglected by dentists thus leading to irreversible effects. In aesthetic dental clinic “Prizma” many dental operations refer to treatment and management of patients with pathological abrasion. We focus on minimal invasive rehabilitation of patients by means of early notification of the necessity of treatment of this abnormality at early stages, so that to avoid even more loss of tooth substance.
Perhaps, not everyone can clearly realize importance of this problem without having immediately visualized it. If you feel sensitivity along chewing and cutting edges after both cold and sweet, observe grinding of teeth, especially at nights (bruxismus), teeth squeezing (especially during distress); chewing muscles hypertension, their permanent strain (in sportsmen as often as not); clicking and delicacy of temporo-mandibular joint — they are symptoms of pathological abrasion. Objectively, tooth crown height is reduced, usuras are visible, faceted surfaces, dentine is bared. And they are only esthetic features. Functionally, joint crepitus appear, vertical dimension if reduced involving appearance of expression lines, genian wrinkles, turning down of mouth angles.
If the problem of pathological abrasion is not solved at initial stages, it entails critical destruction of dental tissues: pulpa and tooth root get damaged, the situation becomes sticky that makes minimally invasive treatment impossible. Our task is to preserve teeth vitality (teeth will keep alive).
Methods of solving of pathologic abrasion problems should have integrative approach and a range of operations. From the beginning, it is a detailed diagnosis of joints with a help of computer tomography and gnathological investigation where patient’s reprints are taken, the models are produced and picture available is examined; and then lost occlusal vertical dimension is restored in articulator and the tooth form desirable after treatment is modeled in wax. Here we have future occlusion and patient’s teeth appearance. And only after detailed diagnosis a right choice of treatment methods becomes possible. In our department we widely use reconstruction of a lost height by renewal with a light curing material, when teeth involved in a process are scarcely treated. Only in a few visits we will not only restore teeth, but sanitize the entire occlusion. A solution is in the smallest interventions on dental tissues. The principal result — teeth remain vivid with preservation of major tissues. Also, orthopedic treatment methods are possible with the use of aesthetic types of prostheses (dental veneers, crowns). After treatment patients are closely followed-up and undergo regular medical check-up.
