What are oral frenulums? Does everyone have them and where?

The frenulum of the oral cavity is an elastic cord of the mucous membrane. We all have three frenulums in our mouths:

  • frenulum of the upper lip – connects the upper lip to the upper jaw. Normally, it is attached at a distance of 5-8 mm from the necks of the anterior incisors. In another case, the frenulum of the upper lip is often woven between the upper central incisors and forms a gap between them (diastema);
  • frenulum of the lower lip – connects the lower jaw and lower lip. Normally, the frenulum is woven into the gum of the lower jaw below the level of the central incisors;
  • frenulum of the tongue – is woven approximately in the middle of the tongue, and its length is about 8 mm. If the frenulum is attached almost to the tip of the tongue – a direct indication for correction.

If the bridle is short? What does the wrong attachment of the bridle lead to.

  1. Violation of the sucking and swallowing reflex in newborns is a consequence of the short attachment of the frenulum of the tongue and upper lip. This condition should be diagnosed immediately after birth. Otherwise, the baby will not receive enough nutrition and will not be able to properly take the breast for sucking.
  2. Violation of the formation of speech – a shortened frenulum of the tongue affects the pronunciation of some sounds.
  3. The presence of a gap between the central incisors of the upper jaw due to the dense mucous interlacing of the frenulum between them, which creates tension on the soft tissues, gums and recessions can occur.
  4. The development of periodontitis – short frenulums of the upper and lower lips pull back the mucous membrane and form a gingival pocket, where microorganisms develop and bone tissue is destroyed in the future. The roots of the teeth can also be exposed, which leads to increased sensitivity.

What are the indications for frenuloplasty?

  1. The presence of a gap (gap) between the upper central incisors due to the interweaving of a thick strand of the frenulum of the upper lip, which prevents the teeth from converging to the center and creates tension on the soft tissues, gums, causing recessions.
  2. As an orthodontic indication to prepare for the installation of braces, since short frenulums play a large role in the formation of malocclusion.
  3. To prevent the development of gum disease or in the process of treating it. In such cases, a short frenulum pulls the mucous membrane, exposes the roots of the teeth and forms gum pockets.
  4. In preparation for prosthetics.
  5. In the direction of a speech therapist, if a short frenulum affects the violation of sound production.

When is the best time to do a frenulum correction?

  • Infants strictly according to the indications, if the frenulums affect the violations of sucking and swallowing, a frenulotomy is performed – dissection of a thin transparent strand of the frenulum to lengthen it.
  • Children aged 8-10 years undergo a frenuloectomy – removal of the frenulum, after the eruption of the permanent upper central incisors, during the eruption of the lateral incisors, or even fangs. Not earlier!
  • In adolescence and older age according to the indications listed above.
